Oral Health Pilot Collaborative. Two health centers: Grace Med Health Clinic and Health Ministries Clinic have participated in the oral health pilot collaborative. The main goal of the collaborative is to promote integration of oral health and primary care, streamline the medical/dental referral process, and improve the oral health of children seen at the safety net clinics. The oral health collaborative uses the Care Model as a framework for quality of care. The oral health pilot collaborative offered on-site and hands-on learning sessions, specific integration strategies between primary care and oral health, and training to medical staff on how to conduct oral health assessment and apply fluoride varnishes. The oral health collaborative focuses on children ages 0-6, although children up to 18 years can also participate in the project.
